Who is responsible

Information you provide
If you contact us by email, telephone, the complaints form, or the appointment request form, we receive the information you choose to provide, such as your name, contact details, appointment request, and message. Please avoid sending unnecessary medical information by ordinary email.
Technical information and cookies
Our hosting provider processes technical request data, such as an IP address, browser or device information, and requested pages, to deliver and secure the website. A functional NEXT_LOCALE cookie remembers your selected language for up to one year. Google Analytics does not load until you accept analytics. Your choice is stored in your browser’s localStorage until you change it or clear your browser data.
How and why information is used
We use information to answer enquiries, arrange or provide care, handle complaints, meet professional and legal obligations, secure the website, and understand aggregate website use. The lawful basis depends on the purpose and may be steps taken at your request, provision of healthcare, a legal obligation, our legitimate interests in operating a secure website, or consent where required.
Website analytics
We use Vercel Web Analytics for aggregate website-use information and ordinary contact-link selections. With your consent, Google Analytics also measures website use and campaign performance. Google advertising storage, advertising user-data sharing, advertising personalisation, and Google Signals remain disabled. You can accept, decline, or change this choice at any time using Privacy choices. Google Analytics does not load on the contact and complaints page, and we do not record a complaint-form conversion event.
Sharing
Information may be shared with service providers needed to operate the website and communications; with another healthcare provider or insurer when needed for care and supported by your consent or another lawful basis; or where disclosure is required by law. We do not sell personal information.
Retention and international processing
Information is kept only as long as needed for its purpose and applicable professional or legal duties. Medical records are retained under Dutch healthcare rules. Technology providers may process data outside the European Economic Area subject to the safeguards required by data-protection law.
Your rights
You may ask to access, correct, restrict, or delete personal information where the law allows. Contact paula@fysioduarte.nl. You may also raise a concern with the Dutch Data Protection Authority.
